Kindergarten Registration – UPDATE

All Dyer County schools will hold Kindergarten registration day on Wednesday, May 2. Fifth Consolidated’s registration will start at 8:30 and last until 2:30.

Please bring the following with you on registration day:

1. Your child’s birth certificate, (must be 5 years old on or before September 30, 2012)
2. Your child’s social security card
3. The new Certificate of Immunization form, which is the child’s immunization record and physical examination record as well as a religious exemption portion
4. You MUST bring proof of residence! This can be either in the form of an electric, gas, or water bill.  No other form will be accepted.
